Support Worker

Sounds great, what will I be doing? As a Support Worker, you will provide person-centred support to service users by carrying out comprehensive assessments, support planning, and safety planning that promote recovery, independence, and individual wellbeing. Working closely with the Service Manager, Team Manager, external agencies, families, carers, and other key supporters, you will help develop and review support, risk, and move-on plans tailored to each individual''s needs. You will encourage participation in social, educational, training, employment, and skills-development opportunities, while supporting access to relevant health services. You will also help maintain a safe and positive environment, monitor housing-related responsibilities, and promote tenancy sustainment, including supporting occupancy agreement compliance and rent payments.
