In this rewarding role, you will work closely with service users to deliver person-centred, strengths-based support that promotes recovery, independence, and wellbeing. You will engage individuals in comprehensive assessments, support planning, and safety planning, ensuring that all plans are user-led and tailored to their unique needs, aspirations, and goals. Working collaboratively with the Service Manager/Team Manager, you will assess referrals and develop, review, and monitor support, safety, and move-on plans.
