The Role As a Support Worker, you will: Provide person-centred care and support in line with individual care plans. Support individuals with personal care, medication and daily living. Encourage independence, choice and decision-making. Support individuals to access the community and develop social relationships. Promote healthy lifestyles and wellbeing. Work closely with Team Leaders, Care Managers and other professionals. Contribute to person-centred plans, reviews and support goals. Act as a Key Worker where required.
