Job ResponsibilitiesParticipate as part of a team managing a new and dynamic service supporting individuals with complex needs, including mental health, physical health, and substance misuse issues.Assess and manage risks for clients and the service, ensuring safety and well-being at all times.Deliver high-quality care, support, and housing management in accordance with organisational policies and procedures.Work collaboratively with partner agencies to provide holistic, wrap-around support tailored to individual needs.Develop and implement client-centred support and risk management plans, aiming for suitable move-on options within six weeks.Engage with clients to build positive relationships, motivate progress, and encourage participation in their recovery journey.Manage challenging situations ..... full job details .....
