The Role
We are looking for a compassionate and motivated Support Practitioner to join our growing team. You will work alongside individuals who may have learning disabilities, autism, mental health needs, or other support requirements. Your role will be to provide person-centred support that helps people build confidence, develop life skills, and engage in their community whilst accessing our vibrant group based support service.
Key Responsibilities

Deliver tailored support to meet each person’s individual needs and goals within a small group setting

Assist with developing and maintaining daily living skills, from cooking and budgeting to personal care

Support individuals to access education, training, work, and social activities

Support individuals to be an active participant in community

Build trusting relationships and act as a positive role model

Keep accurate records and contribute to support planning

Promote independence, dignity, and choice at all times

Ability to support Members Travel to and from Support and encourage the development of self travel skills
About You
-
Caring, patient, and respectful in your approach
-
Good communication and listening skills
-
Able to work as part of a team and on your own initiative
-
Flexible and reliable, with a positive outlook

Experience in a support role qualified to SVQ level 3
-
Experienced in implementing positive behaviour plans
-
Experienced at delivering group based activities that support learning and developement
-
A valid driving licence is desirable but not essential

28 days holiday per year (This will Increase by 2 days after 2 years in service, then by 1 additional day yearly, to a total of 38 days)
