This is a rewarding and varied role where no two days are the same. You will work closely with customers to help them build confidence, maintain independence, and access the right support services. This may include helping individuals move into more suitable accommodation, supporting during difficult times, or simply being a consistent and trusted point of contact. You will work collaboratively with health professionals, social care teams, housing services, and other partner organisations to ensure customers receive holistic and effective support.
