Duties & Responsibilities as a Support and Wellbeing Worker: Provide person-centred support to vulnerable adults, promoting independence, dignity, and wellbeing. Build positive relationships with service users to understand their individual needs, preferences, and goals. Develop, review, and maintain support plans and care plans, ensuring they reflect everyone’s changing needs and circumstances. Complete annual support plan reviews and contribute to multi-agency meetings where required.
