We are currently recruiting a Support and Wellbeing Worker for our client based in Haringey, the post holder will deliver a need led service both in supported Housing schemes and within the community.   Duties &amp; Responsibilities as a Support and Wellbeing Worker:   	 Provide person-centred support to vulnerable adults, promoting independence, dignity, and wellbeing. 	 Build positive relationships with service users to understand their individual needs, preferences, and goals. 	 Develop, review, and maintain support plans and care plans, ensuring they reflect everyone’s changing needs and circumstances. 	 Complete annual support plan reviews and contribute to multi-agency meetings where required.
