Supervisor (RN), Home Health

BAYADA Home Health Care is currently seeking a

Clinical Manager (RN required)

to join our

Rutland, VT

Medicare Certified Home Health office.

Responsibilities for a Clinical Manager:

Lead, inspire, and motivate others to provide exceptional care.

Work closely with the Rehab Manager (where applicable), Director, and DCO to ensure cohesive management of all field staff.

Review all clinical documentation to ensure high-quality, accurate services.

Coordinate with office team members to assign the most appropriate clinicians to each client.

Develop, implement, and update clients'' individualized care plans based on ongoing assessments.

Coordinate client care, referrals, and act as an internal case manager and client advocate. Communicate with team members regarding client status, care plans, and outcomes.

Participate in the hiring and interviewing of field staff, as applicable.

Represent BAYADA in community and marketing activities, including visits to physician offices and facilities.

Qualifications for a Clinical Manager:

Minimum of 2 years of experience as an RN in Home Health.

Oasis experience required.

At least 2 years of leadership or supervisory experience.

Excellent organizational and communication skills.

Desire to lead, inspire, and motivate others.

Strong knowledge of home health policies and procedures.
