Conduct expert assessments and deliver tailored treatment plans for individuals with traumatic, acquired, or post-surgical hand conditions, using a range of therapeutic approaches to support recovery and function.Establish personalised rehabilitation goals and monitor progress through recognised clinical outcome measures.Provide hand therapy services for patients referred by specialist hand consultants, fracture clinic, and rheumatology teams, ensuring timely and effective care.Communicate clearly and compassionately with patients and carers, using both verbal and non-verbal techniques to obtain informed consent, guide rehabilitation, and manage patient expectations.Work collaboratively with colleagues and external partners to ensure seamless and coordinated patient care.Homerton Healthcare NHS Foundation Trust is a high performing and flourishing organisation serving the population of Hackney, The City and beyond.We are extremely proud of the fact that Homerton Hospital has recently attained the highest possible rating of "Outstanding" from the Care Quality Commission (CQC).Our integrated Trust provides comprehensive hospital and community health services for its local population and a range of specialist services for a wider group of patients.
