Main duties

You will work closely with the Principal Pharmacist to provide comprehensive medication reviews for elderly inpatients, supporting safe and effective medicine use to improve outcomes, reduce inappropriate polypharmacy, and enhance patient understanding of medicines.

Deliver ward-based clinical pharmacy services within elderly care, provide clinical support to the dispensary, and gain experience in supervising junior staff, quality improvement and audit, medicines optimisation and safety initiatives at ward level, and collaboration with the Principal Pharmacist on elderly care medicines issues.

Help deliver, develop and evaluate pharmaceutical services for Divisional Teams and Clinical Service Units, meeting the requirements of pharmacy customers in line with legislation, national guidance and departmental objectives.

Deputise for the Principal Pharmacists at meetings and in training within their areas of pharmaceutical care.

Provide ward-based clinical pharmacy services alongside other specialists and technicians, supporting staff and trainees.

Person specificationQualifications and knowledge

B.Sc. / B.Pharm. / M.Pharm

GPhC registration

Postgraduate Certificate in Clinical Pharmacy (or equivalent)

Evidence of ongoing CPD (in line with GPhC requirements)

Experience

Post-registration hospital-based clinical experience

Experience in a multi-disciplinary team and clinical audit

Experience with Medicines Information and knowledge of medicines management at interfaces between primary, secondary and tertiary care

Skills

Effective and safe pharmaceutical skills; strong time management, prioritisation, and organisational abilities

IT skills (computer literacy, word processing, spreadsheets); excellent verbal and written communication; good presentation skills

Leadership potential and ability to motivate and act as a role model
