Specialist Community Palliative Care Social Worker

Overview
We are recruiting a Band 7 Specialist Community Palliative Care Social Worker for our community palliative care team in Camden and Islington. We are looking for someone who can show resilience and motivation to work in this area, and also demonstrate compassion, self-knowledge, confidence, generosity, and courage. Working in the community, you will work as an autonomous practitioner, as part of a team managing a caseload of palliative care patients and their carers and families with a variety of conditions, often with complex needs. The post holder will work as part of the established Camden and Islington Community palliative care team across Camden and Islington boroughs. The successful candidate will provide specialist social work input for patients and families facing life-limiting illness and bereavement, including needs of the family before and after death, and for those living with serious illness, impending death, loss and grief. The role involves independently assessing, planning, and managing a caseload of patients and their families, working collaboratively with other healthcare providers to ensure holistic and coordinated care. They will act as a specialist resource for consultation and teaching within the trust and outside.Responsibilities
Assess, plan, and manage a caseload of palliative care patients and their families, working autonomously within a multidisciplinary team.Provide specialist social work input for patients and families facing life-limiting illness and bereavement, including pre- and post-death needs.Collaborate with other healthcare providers to ensure holistic and coordinated care.Act as a specialist resource for consultation and teaching within the trust and externally.Support the well-being and development of colleagues within a multi-professional palliative care service.About the Team and Settings
As a service we provide palliative care to the patients of Camden and Islington. The community team is based at St Pancras Hospital and has evolved into a multi-professional team of nearly 30 staff, including CNSs, Consultant Nurse, Drs, Social Workers, OTs, Physiotherapists and administrative colleagues.
