Children With Disability Social Workers in Wolverhampton

Purpose of the position: assesses the needs of children with disabilities and their families, develops care plans, and facilitates access to relevant services to enhance their well-being and independence.

Key responsibilities:

• Conducting comprehensive assessments of children with disabilities and their families.
• Developing and implementing care plans that meet the individual needs of children.
• Providing ongoing support and guidance to families, ensuring they have access to necessary resources.
• Working collaboratively with multi-agency teams to ensure holistic support for children.
• Maintaining accurate and up-to-date case records in line with statutory requirements.

Required Children with Disabilities experience.

This is a full time temporary position on ongoing basis.
