Social Worker – West Locality | Adult Social Care

Pay Rate: £28.40 per hour Umbrella
Working Pattern: Hybrid
Office Days: Monday & Friday
Hours: Monday–Thursday 8:45am–5:15pm | Friday 8:45am–4:45pm

About the Role

We are seeking an experienced Adult Social Worker to join the West Locality team, supporting adults with care and support needs and their carers to achieve positive outcomes, maintain independence and improve wellbeing.

The successful candidate will have a strong background in Adult Social Care, with experience in safeguarding, mental capacity assessments, risk management and reablement/enablement approaches.

Key Responsibilities
  • Complete strengths-based Care Act assessments and care planning.

  • Develop, implement and review person-centred care and support plans.

  • Manage a varied and complex caseload within statutory timescales.

  • Undertake safeguarding enquiries and manage risks effectively.

  • Complete Mental Capacity Assessments and contribute to best-interest decisions.

  • Develop and implement reablement and enablement strategies to promote independence.

  • Advocate for adults and ensure their voices, wishes and outcomes remain central.

  • Work collaboratively with families, carers and multi-disciplinary partner agencies.

  • Monitor and review care packages to ensure they remain appropriate.

  • Maintain accurate and timely case records.

  • Participate in supervision and escalate significant concerns appropriately.

  • Support less experienced Social Workers, students and newly qualified practitioners where required.

Candidate Requirements

The ideal candidate will have:

  • A recognised Social Work qualification.

  • Current Social Work England registration.

  • Predominantly Adult Social Care experience.

  • Strong experience of adult safeguarding.

  • Experience completing Mental Capacity Assessments.

  • Strong risk assessment and management skills.

  • Experience using reablement/enablement approaches.

  • Strong Care Act knowledge and assessment experience.

  • Excellent communication and multi-agency working skills.

  • Ability to manage a caseload independently.
