The successful candidate will have a strong background in Adult Social Care, with experience in safeguarding, mental capacity assessments, risk management and reablement/enablement approaches. Key Responsibilities   Complete strengths-based Care Act assessments and care planning.   Develop, implement and review person-centred care and support plans.   Manage a varied and complex caseload within statutory timescales.   Undertake safeguarding enquiries and manage risks effectively.
