      <description>Location: Walthamstow | Salary: &amp;pound;35/hour | Type: Contract | SW Locums require a UK experienced Social Worker to work within Transition Support Service, Community Services in Waltham Forest. Transition social worker to work in a 0-25 Service Team. must have Learning Disability experience.  To work across the boundaries of Children’s and Adult Services to enable a joined up approach to the management of transition planning and in supporting young people with disabilities to develop their maximum potential.  You must hold right to work in the UK and registration with Social Work England.  Your role:  	 To develop transition planning for young people who are eligible for services and support from Adult Health and Social Care services. 	 To ensure that there are effective arrangements in place which support a smooth transition for young people with disabilities from Children’s Services to Adult Health and Social Care provision.
