Social Worker - Short Breaks Team

Connect2Halton is currently recruiting for an experienced Qualified Social Worker to join the Short Break Service for Children with Disabilities Team within Halton Council. We are looking to appoint a specialist Social Worker who shares our commitment to provide services that enable families of children with disabilities to have an improved quality of life within their communities. You will have a key role in assessing the needs of disabled children who have complex needs, producing a robust, person centred, Child in Need plan and supporting them to access short break services.Key Responsibilities:Manage a caseload of children and young people with disabilities in line with statutory requirements and departmental policies.Undertake assessments, care planning, and reviews to ensure individual needs are met effectively.Work in partnership with families, carers, education, health professionals, and other agencies to deliver coordinated support.Promote safeguarding and ensure all practice aligns with relevant legislation and guidance.Support children and families to access short break services that meet their needs and preferences.Qualifications and Experience:Social Work England registration.3 years perm experience.Previous experience of working with children with disabilities.Ability to work under pressure and manage caseloads.Connect2Halton is a trading style of Halton and Kent Commercial Services LLP- A joint venture between Halton Borough Council and Commercial Services ..... full job details .....
