Social Worker - Prevention and Wellbeing

Halton Borough Council are looking for an experienced and passionate Adult Social Worker to join their Prevention and Wellbeing Team. This is a fantastic opportunity to be part of a proactive and supportive service that focuses on early intervention and enabling people to live healthier, more independent lives. Key Responsibilities: Carry out comprehensive needs assessments and create personalised support plans in accordance with the Care Act.Apply robust safeguarding knowledge to protect vulnerable adults and manage associated risks.Encourage independence and wellbeing by linking people to suitable services and community support.Advocate for carers'' rights, ensuring their needs are recognised and addressed within care planning.Keep clear, accurate, and high-quality records on case management systems.Collaborate with health, housing, voluntary organisations, and multi-disciplinary teams to provide coordinated and effective support. Requirements: Social Work qualification (BA/MA/DipSW or equivalent).Current registration with Social Work England.Experience in adult social care, particularly in preventative or early intervention work.Excellent knowledge of the Care Act and safeguarding adults.
