Social Worker - Older Adults / Cognitive and Physical Disabilities - Leicestershire County

Our client, Leicestershire County Council, is seeking an agency social worker for its Cognitive and Physical Disabilities team.This is mainly working with Older Adults and covering the Charnwood/Loughborough area.Pay rate £34 umbrella.You will usually attend the office for your duty day, otherwise you can work from home when not attending visits and meetings in the local area.The role will involve the following:The team provides support for older people with eligible needs related to frailty and cognitive disabilities, arising from long term conditions such as Dementia, Stroke, Parkinson''s Disease and Brain Injury, as well as adults over the age of 18 with physical disabilities.Undertaking Care Act assessments and support planningUndertaking Safeguarding enquiriesDealing with complex situations with people and familiesUndertaking Mental capacity assessmentsBest interests decisionsContinuing healthcareTaking part in duty rotaCourt of Protection work may be involvedPlease let me know if you are interested?
