The SMART service provides practical and psycho-social support for adults who have complex and intersecting needs. The service works with individuals who may experience a cycle of homelessness, mental health admissions, drug and alcohol dependencies, and contact with criminal justice and probation services. The service supports a particularly vulnerable group who are often multiply excluded from traditional services because of the complexity of their needs and the difficulties they may experience engaging with conventional service models. About the Service The SMART service is designed to bridge gaps between different agencies and services by providing intensive, flexible and coordinated support. The team works with individuals who may have a very low quality of life and significant safeguarding risks, including risks relating to neglect, abuse and serious harm.
