Ensure the service user and carer have opportunity to have the shortest length of stay in institutional environment as possible. Discharge will be efficient and effective, with appropriate personalised support ensuring user can be maintained at home and carer is considered and supported within any package of care, ensuring individual flexibility for support where possible and personalised planning aided by VSC support worker is completed.Key contacts
