Social Worker - Learning Disabilities Team

Our client Swindon Council is looking for a Social Worker to join their Learning Disabilities Team.Job PurposeTo provide high-quality, person-centred social work services to adults with learning disabilities, promoting independence, wellbeing, choice, and inclusion. The post holder will assess eligible needs, develop outcome-focused support plans, undertake safeguarding responsibilities, and work collaboratively with individuals, families, carers, and partner agencies to achieve positive outcomes in accordance with statutory duties.Key ResponsibilitiesManage a caseload of adults with learning disabilities, ensuring timely assessment, support planning, and review.Undertake strengths-based assessments under the Care Act 2014, identifying eligible needs and promoting independence.Complete Mental Capacity Act (MCA) assessments and Best Interests decision-making where appropriate.Undertake safeguarding enquiries in line with Section 42 of the Care Act and local safeguarding procedures.Develop person-centred care and support plans that maximise independence and community participation.Carry out risk assessments and implement effective risk management plans while promoting positive risk-taking.Work in partnership with individuals, families, carers, advocates, health professionals, housing providers, and voluntary organisations.Support transitions from Children''s Services to Adult Social Care where required.Participate in Continuing Healthcare (CHC) assessments and ..... full job details .....
