Job Title: Social Worker - Hospital Team - Kirklees
Location: Kirklees (Hybrid)
Pay Rate: -35 per hour

What will your responsibilities be?
In this role, you will support individuals following hospital discharge by conducting timely reviews and participating in Decision Support Tool (DST) meetings to assess ongoing care needs. You may also be involved in Adult Safeguarding work and, after gaining experience, will be expected to contribute to the team''s duty rota, handling urgent cases and initial assessments as part of your responsibilities.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
