To support hospital discharges with people with complex social care needs but also to complete the follow up reviews within the community as well. The team has a mixture of Social Care Assessors, Social Workers, MDT including nurses and health. You must be able to work independently and within a team, and have a high standard of analytical, assessment, organisational and communication skills. You will need to have the ability to prioritise referrals based your own risk assessments and have the personal qualities to maintain the ability to make professional and sound decisions whilst working under pressure.
