Social Worker-Hospital Discharge Team -Derriford Hospital-Devon

Social Worker required for work within the Hospital Discharge Team as a Social worker in the Plymouth Derriford Hospital of Devon. Competitive rates of pay up to -35ph. Full and part time hours considered. Flexible hybrid working. Plymouth is a vibrant city on the South coast and very close to breathtaking moorland.

To support hospital discharges with people with complex social care needs but also to complete the follow up reviews within the community as well.

The team has a mixture of Social Care Assessors, Social Workers, MDT including nurses and health.

You must be able to work independently and within a team, and have a high standard of analytical, assessment, organisational and communication skills. You will need to have the ability to prioritise referrals based your own risk assessments and have the personal qualities to maintain the ability to make professional and sound decisions whilst working under pressure.

You will be Social Work England registered, ideally a driver with access to own car and able to reside and work in the UK.
