      <description>Location: Cosham | Salary: &amp;pound;38/hour | Type: Temporary | Adults Hospital Discharge Social Worker    Locum    -38 an Hour    6 Month Initial Contract    Hampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Queen Aexander Hospital, Portsmouth    Role will be primarily  duty  which will include, triaging, resolving issues in a fast paced environment, safeguarding, managing expectations, speaking with patients and their NOK, managing a task list.  Responsibilities:   Working with the Advanced Assessment Team(AAT).  This individual will be part of a multidisciplinary "sub team" Supporting with early intervention in the hospital to reduce Length of stay.   The individual will be required to support in the following areas (not an exhaustive list):   Completion of care act assessment and Mental capacity act assessments as needed  Building professional MDT networks with key areas in the hospital  Risk assessment work and close liaison with community teams around "High intensity users" and turning these individuals around  Close links with out Short-Term Service and community bed providers to stream people rapidly to the services to avoid admissions and truncate stays.
