Adults Hospital Discharge Social WorkerLocum 38 an Hour6 Month Initial ContractHampshire County Council are recruiting for a Social Worker in their Hospital Discharge Teams based out of Royal Winchester County Hospital.Responsibilities:Working with the Advanced Assessment Team(AAT).This individual will be part of a multidisciplinary "sub team" Supporting with early intervention in the hospital to reduce Length of stay.The individual will be required to support in the following areas (not an exhaustive list):Completion of care act assessment and Mental capacity act assessments as neededBuilding professional MDT networks with key areas in the hospitalRisk assessment work and close liaison with community teams around "High intensity users" and turning these individuals aroundClose links with out Short-Term Service and community bed providers to stream people rapidly to the services to avoid admissions and truncate stays.Use of HCC systems to provide AAT with collateral information to enable rapid decision makingRepresent AAT on wards, as required including participation on Stranded patient session if required.Benefits:A 9 day working fortnightA hybrid tablet device, so you can work more effectively and flexibly.5 personal development days per annum.Access to permanent vacancies as they are released.Flexible working options to fit around today''s lifestyle37 Hours a week 38 Per hour (Umbrella Rate)About UsConnect2Hampshire is a managed service agency owned by Hampshire County ..... full job details .....
