You will need to have experience of hospital social work, including the Home First approach is desirable. Duties include strengths based assessment and support planning , mental capacity act assessments/ best interest decision making . You will be part of the duty rota. The role is largely onsite due to the nature of the work. You will be joining a friendly team, with good manager presence and support. Key Responsibilities: Work collaboratively to ensure safe and efficient hospital discharges. Manage a complex caseload, working independently and efficiently. Provide support and intervention for individuals and families during transition from hospital to community-based care. Liaise with multidisciplinary teams to ensure seamless and person-centered care. About You: Previous experience working in an Integrated Hospital Discharge Team. The ability to hit the ground running in a fast-paced environment.
