      <description>Location: Bath | Salary: 35.00-35.00 Hourly | Type: Temporary | Social Worker required for work within BANES Discharge to Assess Team. 37 hours a week with rates up to 35-38 p/h for experienced candidates. Contract will be minimum 3 months. Flexible, hybrid working.   We are looking for compassionate, proactive, and motivated individuals to join the Discharge to Assess (D2A) Team across Bath and North East Somerset (BANES).   The D2A pathway supports individuals to leave hospital safely and continue their assessment and recovery in the most appropriate setting, helping people regain independence and avoid unnecessary long-term care.   Our D2A Team works closely with health, social care, therapy, and community partners to support safe hospital discharge and ongoing recovery within community settings.
