We are looking for compassionate, proactive, and motivated individuals to join the Discharge to Assess (D2A) Team across Bath & North East Somerset (BANES). The D2A pathway supports individuals to leave hospital safely and continue their assessment and recovery in the most appropriate setting, helping people regain independence and avoid unnecessary long-term care. Our D2A Team works closely with health, social care, therapy, and community partners to support safe hospital discharge and ongoing recovery within community settings. Team members play a key role in helping people regain independence and achieve the best possible outcomes following discharge from hospital.
