This is an excellent opportunity to work within a well-structured team dedicated to supporting children with disabilities and their families. Role and Responsibilities Carry out statutory assessments under the Children Act 1989, including Child in Need and Child Protection assessments. Develop and implement care and support plans that meet the individual needs of children with disabilities. Work collaboratively with families, education providers, health professionals, and other agencies to ensure the best outcomes for children. Conduct safeguarding investigations and contribute to child protection conferences, core group meetings, and Looked After Child reviews where required. Provide direct support to children and young people with disabilities, ensuring their voices are central to all decision-making processes. Manage a complex caseload, ensuring compliance with statutory duties, including Education, Health and Care Plans (EHCPs), direct payments, and specialist services. Support transition planning for young people moving into adult services, ensuring continuity of care and support.
