Social Worker Adults NHS Continuing Healthcare - 12 months FTC

Your new role Southend City Council are looking to appoint a CHC Lead (Qualified Social Worker) within a 12-month fixed term contract.The successful candidate will have in-depth knowledge and expertise in NHS Continuing Healthcare (CHC) processes and frameworks. They will be required to attend and actively participate in Decision Support Tool (DST) meetings, ensuring robust representation of social care perspectives.Provide appropriate professional challenges and disputes where required, supporting fair and evidence-based decision-making. Acting as the consultative resource for social work colleagues, they will offer advice and guidance on CHC matters, support best practice and consistency across Adult Social Care in relation to CHC. The post-holder will report to the Level 9 CHC Lead and work closely with health and social care partners.Within this position, hybrid working opportunities are available.
