You will benefit from a strong supervision culture, flexible working arrangements, and a collaborative team environment.Key ResponsibilitiesWork closely with professionals across health, voluntary and statutory sectors as part of a multi-disciplinary team, ensuring effective communication and shared best practice.Review, monitor and evaluate care plans and the quality of service provision, responding proactively to changing needs.Develop, implement and assess reablement programmes within care packages, promoting independence and positive outcomes.Manage your own caseload or specialist projects, ensuring appropriate interventions and timely outcomes.Complete risk assessments, safe methods of work practice guides, and raise awareness of a wide range of care-related issues and their implications.Undertake visits across the community, including home visits, as required.RequirementsQualified Social Worker with relevant experience in Adult ..... full job details .....
