The successful candidate will work collaboratively with adults, families, carers and partner agencies to assess needs, manage risk and develop appropriate care and support plans. Key Responsibilities 	 	 Undertake  Care Act assessments  and reviews of adults with care and support needs. 	 	 	 Develop person-centred care and support plans focused on strengths, independence and positive outcomes. 	 	 	 Manage an allocated caseload, prioritising complex and safeguarding cases appropriately. 	 	 	 Assess and manage  risk , including responding to safeguarding concerns.
