Social Worker (Adults) Social and Healthcare Qualified Job Reference: JAJF4131 G9 Service Area: Adult Social Care Contract: Locum / Temporary About the Role We are seeking a qualified and experienced Adult Social Worker to support adults with care and support needs, helping individuals maintain their independence, wellbeing and quality of life. The successful candidate will work collaboratively with adults, families, carers and partner agencies to assess needs, manage risk and develop appropriate care and support plans. Key Responsibilities Undertake Care Act assessments and reviews of adults with care and support needs. Develop person-centred care and support plans focused on strengths, independence and positive outcomes. Manage an allocated caseload, prioritising complex and safeguarding cases appropriately. Assess and manage risk, including responding to safeguarding concerns. Complete Mental Capacity Assessments and contribute to best-interest decision-making where required. Work collaboratively with health, social care and other multi-disciplinary professionals. Coordinate appropriate care packages, support and community services. Promote reablement and independence, wherever appropriate. Advocate for adults and ensure their wishes, views and desired outcomes remain central to planning.
