Job Title: Social Care PractitionerLocation: Shropshire Council Rate: 14.91 PAYE / 19.61 Umbrella Hours: 37 hours per week (Mon-Thurs 8:45-5pm / Fri 8:45-4pm) Contract: 3 months (potential for extension) Role Overview:Opus people solutions are looking for someone to join the Adult Social Care team to support the delivery of person-centred services aligned with the Care Act 2014, the Council''s values, and an early intervention and prevention model.You''ll work with adults including older people, individuals with physical or learning disabilities, sensory impairments, and their carers-helping them achieve independence through assessments, planning, and access to services. Desirable - Driving Licence and access to own vehicle. Key Responsibilities:Deliver assessments and support plans in line with the Care Act 2014Promote early intervention, prevention, and strengths-based approachesSupport timely hospital dischargesWork collaboratively with professionals to assess need and coordinate careEnsure accurate records, attend meetings, and represent the servicePromote equality, anti-discriminatory practice, and health and safety standardsParticipate in rotas (e.g., emergency/same-day responses) Essential Skills and Experience:Experience working with older adults, individuals with disabilities, and carersKnowledge of the Care Act 2014, personal budgets, and self-directed supportUnderstanding of reablement, assistive technology, and person-centred modelsAbility to work across ..... full job details .....
