Social Care FacilitatorLocation: Walsall CouncilRate of pay: £18ph PAYEWorking Pattern: 37 hoursWorking Structure: Hybrid - 2 mandatory days in the officeDuration: 3 MonthsEssential requirements: A DBS check may be applicable for this role and DriverOpus people solutions are currently recruiting for a Social Care Facilitator for our client based in Walsall on a temporary on-going basis for 3 months.About the RoleYou will work as part of a multi-skilled team responsible for meeting the assessment, care management and support needs of vulnerable adults and their carers.The role focuses on enabling individuals to exercise choice, maintain independence and access appropriate community resources, while promoting health, wellbeing, and person-centred outcomes. You will contribute to assessments, support planning and care management activities in line with legislation, including the Mental Capacity Act, and Council policies.What We''re Looking For:Experience or strong understanding of working with vulnerable adults and carers within a health or social care settingKnowledge of person-centred approaches, self-directed support and promoting independenceAbility to communicate effectively with service users, families, carers and professionalsUnderstanding of safeguarding, equality, diversity and anti-discriminatory practiceAwareness of the Mental Capacity Act and its application in practiceStrong organisational skills, with the ability to manage casework and maintain accurate ..... full job details .....
