You ll oversee the delivery of person-centred support planning, ensuring services meet the highest standards and respond effectively to customer needs. By fostering strong partnerships with third-party agencies, you ll help create a holistic, 360-degree approach to support. You ll also take ownership of performance monitoring, reporting key indicators, and maintaining strong relationships with commissioning teams.
