One service provides Step Down accommodation for individuals discharged from hospital, helping them transition safely into the community. The second provides temporary accommodation and support for individuals preparing for independent living or requiring emergency housing provision.Working as part of a small, supportive, and dynamic team, you will play a key role in providing high-quality support, managing complex cases, and helping individuals move towards stability and independence.This is a rewarding opportunity for someone passionate about supporting vulnerable people and making a genuine difference in the community.Key ResponsibilitiesAssess, monitor, and manage risk for individuals accessing the serviceReview referrals, assess suitability, and coordinate client admissionsDeliver high-quality support and housing management in line with organisational policiesDevelop ..... full job details .....
