Overview

Purpose: Manage a caseload of complex mental health clients within the Older Adult Service, delivering health and social care interventions to support recovery, independence, and safeguarding.Responsibilities

Manage complex cases within the Older Adult Service.Commission care packages and ensure appropriate service provision.Undertake safeguarding investigations and risk assessments.Monitor mental state, identifying relapse and supporting recovery.Participate in the Central AMHP rota, providing Mental Health Act assessments and coordination.Work collaboratively with health, social care, and external agencies.Skills and Qualifications

Qualified and registered Social Worker with AMHP status.Experience in mental health, safeguarding, and complex case management.Strong knowledge of the Mental Health Act and related legislation.Excellent assessment, communication, and multi-agency working skills.Additional Requirement
