The role will involve the following: UASC post is for a 16 plus UASC and Pathways Team, therefore a combined role Maintaining your personal performance with the support of your Team Manager. Undertaking a range of assessments informed by direct intervention with children and families. Progressing children''s care and pathway plans, by ensuring that they are accessing the right level of service at the right time. Undertaking statutory visits to children placed at home, in foster care, and within residential provisions in accordance with CLA requirements. Case management and co-ordinating a range of meetings including care planning, risk management, placement stability, PEP''s, and review meetings. Providing children and families with information and advice to assist in resolving problems. Some court work relating to revocation of care orders, SGO and placement order applications, as required.... Permanent
