Senior Practice Nurse Prescriber

Overview

The Greenhouse is an inclusion practice managed by East London Foundation Trust. We register adult patients who are experiencing homelessness and provide the full range of primary care services, with an outreach service in Hackney and the City delivering clinics in hotels, hostels and shelters for vulnerably housed service users and rough sleepers. The role is based in the practice (1 day) and the outreach hotel settings (2 days).Job Responsibilities

The post holder is responsible for delivering safe and effective nursing care to the practice population.Lead and manage the Health Care Assistant and be accountable for nursing service delivery.Provide evidence-based care for patients with long-term conditions, wound care, prevention and health promotion.Work autonomously with clear clinical decision-making and expert care within general practice.Collaborate with the multi-disciplinary general practice team to meet patient needs, support policy and procedures, and provide nursing leadership.Build effective working relationships with other health care professionals and contribute to the delivery of high-quality primary health care services.Participate in a combination of practice-based and outreach service delivery.As an independent prescriber, prescribe safe, effective and appropriate medication in line with current law and local policies.Maintain accurate, contemporaneous patient records using appropriate computer systems and consider Caldecott Principles for data handling.Qualifications and Requirements
