Senior Case Worker (Hospitals)

Joining us as a Senior Case Worker, you''ll be part of our friendly and supportive team of social care and health care professionals. You''ll be based at Clarence Unit in Gosport, our short-stay Recovery and Assessment suite, for a minimum of three days a week so you can meet with individuals and their families. You may also be needed occasionally to support onsite at Queen Alexandra Hospital in Portsmouth. You''ll enjoy the benefits of engaging with people in a face-to-face environment, working alongside colleagues in a supportive space. There will be plenty of opportunities to collaborate and share learning with peers, whilst building positive relationships. What you''ll be doing: Completing assessments and reviews of individuals'' care and support needs. Providing support and guidance to individuals to maximise their wellbeing and independence using a strengths-based approach. Collaborating with individuals, their families, providers, and other healthcare professionals to find and signpost to innovative, alternative community resources to meet individuals'' needs. Having the confidence and knowledge to make well-informed decisions where appropriate, as well as knowing when a case or situation requires escalation to senior colleagues. What we''re looking for: Strong communication, negotiation and relationship management skills. Ability to work with people in difficult circumstances in a compassionate and collaborative way.
