The Role: Joining us as a Senior Case Worker, you''ll be part of our friendly and supportive team of social care and health care professionals based at Basingstoke Hospital. You''ll also support our short-term service to deliver safe and efficient hospital discharges. What you''ll be doing: Completing assessments and reviews of individuals'' care and support needs in Basingstoke Hospital, local community hospitals and care homes. Providing support and guidance to individuals to maximise their wellbeing and independence using a strengths-based approach. Collaborating with individuals, their families, providers, and other healthcare professionals to find and signpost to innovative, alternative community resources to meet individuals'' needs.
