We have an exciting opportunity for a Senior Care Coordinator to support in our clinical service delivery. Many of our clients have complex needs and this role ensures that clients acquire the support they need to receive multi-agency services. Would you like to join our award-winning organisation? About the role The role involves managing the care coordination service area assessment and organising the allocation of care-coordination tasks, working alongside the Adult Team Manager and Clinical Services Manager as part of the intake panel and holding a caseload offering care coordination to survivors of torture. This role ensures that service users at Freedom from Torture have a central role in planning their care planning and are supported to access the most appropriate support for their needs. Working together with our therapists, and legal and welfare staff as part of a multi-disciplinary team, the ideal candidate will be a Registered Mental Health Nurse, Social Worker or Occupational Therapist with significant experience of working in the mental health sector with complex mental health presentations and will have a ..... full job details .....
