Role: Social Worker Adult Mental Health Department: Adult Social Care, Stockton-on-Tees Borough Council Location: Stockton-on-Tees Purpose of Role Social Personnel is advertising a Social Worker role for Stockton-on-Tees Borough Council. We are seeking an experienced and motivated Social Worker to join the Adult Mental Health team. You will play a key role in safeguarding, supporting, and empowering adults with mental health needs, ensuring they receive personalised care and protection in line with statutory duties. Key Responsibilities Conduct strength-based assessments under the Care Act 2014, delivering tailored support plans. Work collaboratively with partner agencies to ensure coordinated and effective interventions. Support individuals detained under the Mental Health Act 1983, ensuring safe discharge and robust Section 117 aftercare plans. Assess eligibility for Continuing Health Care (CHC) and lead Decision Support Tool meetings. Carry out mental capacity assessments and make best interest decisions under the Mental Capacity Act 2005. Engage with the Court of Protection to uphold individuals rights and safeguard wellbeing. Essential Skills and Experience Minimum of two years experience working with adults whose primary need is mental health. Qualified Social Worker registered with Social Work England.
