Duties and Responsibilities Manage an allocated caseload of adults requiring support with hospital discharge and ongoing care needs. Undertake comprehensive Care Act assessments, reviews, and care and support planning. Assess individual needs, risks, strengths, and eligibility and arrange appropriate care and support. Work collaboratively with health professionals to facilitate safe and timely hospital discharge. Undertake Mental Capacity Assessments and Best Interests decision-making where required. Identify and respond appropriately to safeguarding concerns. Work closely with patients, families, carers, health professionals, housing, and other partner agencies.
