Duties and Responsibilities     * Manage an allocated caseload of adults requiring support with hospital discharge and ongoing care needs.   * Undertake comprehensive Care Act assessments, reviews, and care and support planning.   * Assess individual needs, risks, strengths, and eligibility and arrange appropriate care and support.   * Work collaboratively with health professionals to facilitate safe and timely hospital discharge.   * Undertake Mental Capacity Assessments and Best Interests decision-making where required.   * Identify and respond appropriately to safeguarding concerns.   * Work closely with patients, families, carers, health professionals, housing, and other partner agencies.   * Promote independence, prevention, and strengths-based approaches when developing support plans.   * Maintain accurate case recordings, assessments, reviews, and reports in line with statutory requirements.   * Ensure work is completed within required timescales and in accordance with relevant legislation and local procedures. ...</description>
