This service enables patients to safely receive care and monitoring in their own homes through digital healthcare solutions, improving outcomes while reducing hospital admissions.About the RoleAs a Remote Prescribing Pharmacist, you will conduct telephone-based medication reviews with patients enrolled in the service.
