Registered Nurse or Nursing Associate - Diabetes

Overview

We have an exciting opportunity for a Registered Nurse /Nursing Associate to join our growing Diabetes Team. Based at Royal Derby Hospital you will work within the diabetes team supporting our multidisciplinary team (MDT) to provide individualised, holistic care in a safe and caring environment. This will be within the Diabetic foot clinic and the diabetic ward. In discussion with the MDT , the patient and significant others assess the immediate and ongoing needs of the patient and develop an individualised plan of care, implementing and evaluating care input. Providing support to the diabetes team with clinical duties and skills including venepuncture, cannulation and wound management and dressings. Supporting and advising junior members of the team. Collaborating and sharing information within the Diabetes team, UHDB team and where necessary with our Community Partners.Responsibilities

In discussion with the MDT, the patient and significant others assess the immediate and ongoing needs of the patient and develop an individualised plan of care.In consultation with the MDT prioritise, implement and evaluate patient care, in accordance with Trust polices, procedures, protocols and guidelines.Undertake a range of extended clinical procedures relevant to the clinical area, in support of registered nurses and to ensure the implementation of individualised care plans (e.g. wound care and management, dressings and treating diabetic wound and general ward work).Communicate effectively by giving, receiving and interpreting information so that appropriate action can be taken within the boundary of the role.Work in partnership with all members of the interdisciplinary team both internal and external to the Trust to ensure continuity of care for the patient.Reflect on own practice to identify future personal developments. Where appropriate provide information to assist others in their personal development.Team and Collaboration
