Liaise with wards and external agencies to ensure social care packages are in place prior to discharge.Carry out simple discharge referrals to external agencies such as Community nurses and Social Services and contribute to the wider discharge service.Work autonomously across shifts and interact with patients and relatives in a calm and reassuring manner.Demonstrate a caring and compassionate demeanor, good communication skills, and a "Can Do" attitude.Prioritise conflicting demands to deliver patient and service needs.Qualifications

Registered Adult Nurse with a First Level nursing degree (or equivalent) and NMC registration.Additional information
